Answers:

1. The manufacturer has a different "target" in mind. For example, some recommend a smaller box and higher tuning for better power handling. Or the manufacturer's recommendation is for car audio use, where a "maximally flat" design may not produce the best results

2. Basically they're equivalent to filters, e.g. "QB3" is "quasi-third order butterworth".

3. Include the flange in your measurements.

4. The manufacturer usually provides recommendations on how to calculate port length for those types of ports. If not, you might have to find Fb manually and adjust the port length accordingly.
